2017 Annual Dinner Registration

LAHP's 2017 Annual Meeting and Dinner will be held on Thursday, Dec. 14, in The Blue Room at The Roosevelt Hotel New Orleans. The reception begins at 6pm and will be followed by the dinner from 7 - 8:30 p.m.

Please complete the form below to register. After the registration form is submitted, you will be asked to submit payment for your registration fee by credit card. You may skip payment if you are the recipient of a complimentary dinner invitation. Please read the cancellation policy below in bold as it applies to all dinner attendees, including complimentary attendees.

$199.00 (for non-complimentary attendees)

Registration Deadline:

December 5, 2017


Payment is due by December 5th - Registration is not complete until payment is received.

All requests for registration cancellation and refund must be made in writing to jford@lahp.net and submitted no later than Nov. 28, 2017. If you paid with your credit card, the refund will be sent directly to your account. Any cancellations for hotel reservations must be made directly with the hotel. Refunds will not be given for any requests received after Nov. 28, 2017. If you register to attend the dinner as a complimentary attendee without canceling and do not pay or attend, you will be assessed a $199 fee to cover the cost of food for your registration.
